Memory care at Somerfield Health Center in Brentwood, TN offers a safe, comfortable and supportive environment for people dealing with Alzheimer’s and other forms of dementia. All suites are private and can be personalized with family photos and treasured items to make each resident feel right at home.

Our Somerfield Staff get to know each person in our care on an individual level, learn their unique history, and their likes and dislikes. By learning each resident’s story, we’re able to make those heartfelt connections that bring joy and fulfillment to their lives.

It’s the guiding principle behind Heartfelt CONNECTIONS – A Memory Care Program®, a nationally recognized model built on a philosophy that what remains is more important than what’s been lost. Each member of our memory care team is trained in the Heartfelt Connections® program to better address resident needs.

Care plans support individual preferences and are coordinated with our medical team of RNs, nurse practitioners, and medical director. In addition, residents receive respectful assistance with daily living, along with enriching activities coordinated by our Community Life Services Director.

Somerfield offers beautifully landscaped outdoor sitting areas that are enclosed to prevent residents from wandering.

We accept seniors from the surrounding area upon availability.

Residents with LifeCare® will continue to pay their independent living monthly fee after moving to our memory care neighborhood. There may be charges for ancillary items and additional meals.
