What is skilled nursing?
Skilled nursing is for seniors who need more care than assisted living provides, but who don’t require hospitalization. They may be recently discharged from the hospital and need post-acute care to regain their strength and function. Or they may need long-term care for a chronic condition. At The Heritage, we provide around-the-clock skilled nursing care on-site at our top-rated Somerfield Health Center.
Does Somerfield Health Center accept Medicare coverage?
Somerfield is Medicare-certified and will work with you to optimize Medicare coverage and complete documentation to submit for long-term health insurance claims.
Does Somerfield Health Center accept direct admissions?
Depending on availability, Somerfield accepts seniors from the surrounding area who need long-term skilled nursing care. Residents of The Heritage with LifeCare® have priority access to long-term health care services at Somerfield.
Will the monthly fees change for residents moving to skilled nursing?
Residents with LifeCare® will continue to pay their same independent living monthly fee after moving to Somerfield Health Center for skilled nursing, assisted living, or memory care. There may be charges for ancillary items, optional add-on services, or extra meals.
